Planning, coordination, and monitoring of fall prevention programs focused on effective strategies and interventions to help older adults decrease their risk of falling and improve their overall health.

Performs a variety of tasks with emphasis on the delivery of evidence-based falls prevention programs to empower older adults to reduce falls and/or their risk of falls.

Case managers provide a range of services provided to assist and support individuals in developing their skills to gain access to needed medical, behavioral health, housing, employment, social, educational, and other services essential to meeting basic human services; linkages and training for patient served in the use of basic community resources; and monitoring of overall service delivery. The Prevention Specialist is responsible for the programmatic duties in the youth prevention programs (YPU and/or YPI and/or YPS) with the Department of State Health Services (HHSC) grant. Under the supervision of the Youth Programs Director and the coordination of the Prevention Coordinator. More Info
